$______________Fee Enclosed.   Fee is $20 per person.  The fee covers dinner Saturday and lunch Sunday as well as intermediate munchies.  Make checks payable to United Church of Huntsville, designated for 50th anniversary.   Please return this information no later than September 15th.  The caterer requires that he be given the numbers for the Saturday evening meal in advance. Vegetarian meals will be provided if noted above.

If you wish childcare it must be noted above.  For those in childcare, a child appropriate meal will be provided Saturday evening.
